June 3rd Accessible Nature Wellbeing Pilot Program

Thank you for participating in the June 3rd Accessible Nature Wellbeing Pilot Program at Prospect Lake.

Please complete this survey to help us improve and promote the programs we offer. It is important that our work is shaped and led by our community of peers living with disability/ chronic illness.

As a token of appreciation, we will have a random draw of all respondents who complete the survey by June 17th. The person selected will receive $50 and a set of nature photographs that can be used for indoor forest bathing.

By responding, you agree to allow EcoWisdom and partners to use your responses in promotional materials but you can choose if you want your name used or not.
